PHYSICALS FOR STUDENTS
If your child needs a current physical to participate in a team sport and you have been unable to schedule an appointment with your child's family doctor, our school doctor will be at school twice a year to perform routine screening physicals free of charge. The doctor will not be writing any prescriptions, updating immunizations or treating any specific medical problems. This will strictly be a screening physical for participation in sports. It is a state law that any student who wishes to participate in any school sponsored athletic team or squad, must first have a current physical (within 365 days of the first day of the activity). A copy of the completed plysical will be sent home.
Next Date for Physicals in School: Fall, 2020 (date will be posted once scheduled with school physician)
Permission Slip and History Form due by: TBD
Once the dates are confirmed with our school doctor, the permission slip must be signed by the student's parent/guardian and the HISTORY portion of the PPE (preparticipation physical evaluation) must be returned before the physical can be done. Please return the permission slip AND the PPE with the HISTORY portion completed by the due date in order to be scheduled. There are limited spaces available.
