• Forms and Documents

    Below you will find some forms that are necessary if your child:

    * has asthma - doctor and parent must complete the asthma action plan (please bring the inhaler to school in the box from the pharmacy with the child's name on it - please check expiration date)  ** due to Covid-19 - nebulizer treatments will NOT be administered in school due to the possibility of aerosolized spread of the virus

    * has an allergy requiring an epipen or benadryl - doctor and parent must complete the allergy action plan  (please bring the epipen and/or benadryl to school in the box from the pharmacy with the child's name on it - please check expiration date)

    * wants to participate in school sports - doctor and parent/guardian must complete a PPE (preparticipation physical evaluation) within 365 days of the first day of tryouts/participation (parent/guardian completes page 1 --and page 2, if applicable; doctor completes pages 3 and 4).  If PPE was completed 90 days or more prior to the first day of try-outs, then a Health History Update Questionnaire must be completed by the parent/guardian.  Parent and student must also complete the After School Activity Permission Slip.

     *requires medication in school - doctor and parent must complete the medication permission form - required for all over the counter medication also  (please bring the medication to school in its original container - unless regular strength tylenol or ibuprofen which is supplied by the school)
