Forms and Documents

Below you will find some forms that are necessary if your child:

* has asthma - doctor and parent must complete the inhaler release and the doctor must complete the asthma action plan (please bring the inhaler to school in the box from the pharmacy with the child's name on it)

Asthma Action Plan

* has an allergy requiring an epipen or benadryl - doctor must complete the food allergy action plan and doctor and parent must complete the inhaler/epipen/benadryl release (please bring the epipen/benadryl to school in the box from the pharmacy with the child's name on it)

Food Allergy Action Plan

* requires medication in school - doctor and parent must complete the medication release - required for all over the counter medication also  (please bring the medication to school in its original container - unless regular strength tylenol or ibuprofen which is supplied by the school)
